- Referral from your physician
- All pertinent MRI, CT scans, myelogram reports and X-Rays
- Records from other physicians who have treated your pain
- Nerve test results (EMG/NCV, CPT).
- Medications (please bring all bottles)
- Insurance cards and picture ID
